Current or former students who wish to request a JSerra transcript should print, complete, and return the Transcript Request Form below. Please note the following:

Please print legibly and provide all required information. Illegible or incomplete forms will not be processed.

Be sure to include the appropriate fee (Unofficial transcripts—$5.00 per copy Official transcripts—$10.00 unlimited per single school).

Mail or hand-deliver the completed form (and any required fee) to the attention of the JSerra Registrar.

Allow two working days for processing.
